Personal care services will be provided on an hourly basis, tailored to the individual needs of the client. These non-medical personal care, homemaker, and respite care services will be identified, offered, and delivered upon agreement to individuals who are ill, disabled, elderly, infirm, or otherwise in need, enabling them to remain in their own residence in alignment with their desires, abilities, and safety. Clients of Lawana Home Care will receive a "Welcome" package that includes the following:

Client Bill of Rights

• A list of personal care services to be provided by the assigned aide, including the name of the caregiver and supervisor.
• A contact list of personnel responsible for supervising the client’s care, along with the contact information for each individual.

Designated Service Area

• The designated geographical service area of Lawana Home Care will cover King and Snohomish Counties.
• The Lawana Home Care quality improvement plan will apply to all services delivered throughout King and Snohomish Counties.
